This module presents the role of Health Care Networks in caring for people with chronic diseases, addressing definition, etiopathogenesis, physiopathology, clinical manifestations, forms of transmission, forms of prevention, treatment and complications.
Among the topics raised in debate is the identification of two social determinants during the health care process of two patients in accordance with the concepts of a people-centered approach; the ability to organize the work process with a focus on planning and monitoring on hypertensive care; the instrumentalization and creation of therapeutic plans and goals; the importance of recording non-prontuary information for continued care; the organization of the work process in a multiprofessional team, among others.

Understanding the comprehensive and multiprofessional management of Systemic Arterial Hypertension, Diabetes Mellitus and its complications from the Health Care Network in the Basic Care Network.

PÚBLICO-ALVO: Student, teacher, veterinarian or zootechnician, physical education professional, biologist, occupational safety technician, biomedical, odontological, radiology technician, pharmacist, clinical pathology technician, doctor, speech therapist, 2nd cycle medical professional , dental technician, physiotherapist, social worker, nutritionist, technician or nursing assistant, occupational therapist or orthopedist and nurse.
HOURLY LOAD: 48 hours, divided into 06 (three units)
